There is no area of the face that is more likely to show the signs of aging than the eyes. Due to a variety of factors including heredity, environment and age, the upper and lower eyelids are prone to the development of wrinkled skin, discoloration, and puffiness. The eyes are perhaps the most unique identifying facial feature. The best eyelid lift (blepharoplasty) technique must be uniquely tailored to the individual. You should never look different after eyelid lift, rather, you should just look more youthful, refreshed, and rejuvenated. A traditional blepharoplasty is an eyelid lift that might include any or all of the following: removal of excess upper eyelid skin, removal of excess eyelid fat, and removal of excess lower eyelid skin. There are a number of different treatment options, depending on the patient’s needs. Treatment can be focussed on the upper or lower eyelids. About this treatment

Background to the procedure

If just the eyelid surgery is being performed, it usually takes about an hour. However, if you are combining the procedure with any other cosmetic enhancements or facelifts, then it could take longer.

Upper Eyelids; When performing upper eyelid surgery, an incision is hidden within the natural fold of the upper eyelid and extends slightly beyond the outside corner into the laugh lines or other existing creases. Excess skin and fatty tissue are removed through this incision. The incision should heal well because it follows the natural contour of the upper eyelid.

Lower eyelids;  Lower eyelid surgery is performed by making an incision, which is also hidden, but below the lower lashes. The excess skin, muscle, and fat are removed, or fat may be redistributed to eliminate any puffiness or bulges. If muscle laxity is an existing problem, this will be performed also. The incision will heal well in lower eyelid surgery also because the natural creases camouflage it.

Transcongectival lower eyelids;  It is also possible for the plastic surgeon to make an incision inside the lower eyelid to remove the excess fat. This type of eyelid surgery does not require an external incision (Transcutaneous), but it cannot be used to remove excess skin. A laser may be used along with this technique to tighten the lower eyelid skin.

Eyelid surgery can often be done with a local anesthesia, although sometimes general anesthesia is preferred when there are multiple procedures performed. The best option for you will be discussed with the surgeon ahead of time.

What would I expect at the consultation?

All consultations are private one-on-one sessions with Dr. Salman that normally last 20-30 minutes. During that time, the patient and surgeon will discuss their expectations for the results of the eyelid surgery The surgeon will also investigate the medical history of the surgery candidate, to ensure that the procedure is appropriate for the individual.

As part of the process, Dr. Salman will give you the opportunity to examine photographs of patients, so that you can see for yourself what to expect from the surgery. He will discuss with you the range of surgical techniques to get your desired effect and any additional surgical and non-surgical treatments that may be available and of benefit to you.

What is the recovery after eyelid surgery?

Recovery time for eyelid surgery will vary amongst different individuals. Directly following your surgery, you should rest with your head elevated and you may be instructed to apply a cold compress to your eyelids.

Work and daily activities should be avoided for a few days after eyelift surgery. A tight sensation and mild discomfort will result from the surgery, as well as swelling and bruising. Typically the bruising and swelling disappears within 7-10 days, but some patients will find that it is more persistent. Vision may appear blurry for a few days, and an increased sensitivity to lights. Either tearing or dryness may accompany burning or itching feelings. Normal activities can be resumed within ten days or less.

Avoid the sun or any UV exposure for a few months after an eyelift surgery. Always wear UV protective sunglasses. You may also wish to buy the larger frames to help cover up all of the areas around your eyelids. Sun exposure can make your skin look pinkish, red or splotchy and will harm your sensitive eyes.

Some women who want to get an eyelid lift do so in conjunction with other facial procedures. This is a great way to maximise your time, budget and to achieve a brand new look with a minimal healing time.

What are the risks of complication?

What is the difference between Transcutaneous and Transconjunctival techniques?

Transcutaneous literally means through the skin – where an incision is made on the top surface of the skin. With the transconjunctival technique, an incision is made through the inside of the lower eyelid, thereby avoiding an external incision through the skin.

How do I prepare for eyelid surgery?

2 weeks pre-operatively

Stop taking any aspirin or aspirin containing products as these may increase the risk of bleeding. Also any non steroidal anti inflammatory drugs (NSAIDs) such as Brufen or Volterol such should be stopped, as should any Vitamin E tablets or multi vitamins containing vitamin E. Stay away from the sunrays for at least 2 weeks before surgery and 2+ months after. If this cannot be avoided use sunglasses with UV protection and an appropriate sunblock.

1 week pre-operatively

It is advisable to take 1 gm vitamin C twice a day to improve wound healing.

Night before surgery

Prepare ahead with ice packs and medical necessities. Nail polish should be removed prior to surgery for the anaesthetic equipment requirement.

The day of surgery

Don’t use any lotions, cream or makeup on your face, especially eye shadow, mascara, eyeliner or any type of eye care products.
